Support Coordination is a key resource for individuals and families that helps plan for and guide all adult services for an individual eligible for New Jersey DDD services.   A Support Coordinator will work with the individual and their family to begin the person-centered planning process, including the Individualized Service Plan (ISP). Learn about the process of identifying a support coordinator, their roles and responsibilities and explaining the different waivers, tiers and acuity. Registration requested.
