Items they cover include:

1. Mobility equipment
2. Medical equipment and procedures
3. Hearing aids, dentures, eyeglasses
4. Basic needs such as clothing or food for children
5. Short term day care
6. Prescriptions
7. Cobra/health insurance
8. Ramps
9. Van modifications
10. Prosthesis

Application for assistance must be filled out by a social worker or agency.
