A training watch is a great alert system for kids.  http://www.leapsandbounds.com/ has one for $10.95.  You set it to go off at particular times or intervals, and it vibrates or beeps, signaling to child to get to the bathroom.  That we, no one is telling him verbally to go.  An issue in some kids with DS is lower abdominal tone or lessened ennervation, meaning that he gets less of a warning that he has to go and less time to get to the bathroom. Your PT can also suggest specific exercises to strengthen the appropriate muscle groups.  Playing in squat is great for toileting.
